MAST Permit Replacements
WASHINGTON
Easy 4 Step Process
The WSLCB requires you to have your MAST Permit and a valid form of ID readily available for inspection by law enforcement any time you mix, serve, sell or supervise the sale of alcohol at a licensed premises.
Why Should I Replace My MAST Permit?
There are two reasons to replace your MAST Permit:
1. You lost your MAST Permit.
If your permit is lost, stolen or damaged, you will need to replace it as soon as possible. If you are caught working without a MAST Permit, you may receive a citation accompanied by a 5-day permit suspension or $100 fine. Your employer can also receive a citation and a 5-day liquor license suspension or $500 fine. Subsequent offenses result in increased penalties for both you and your employer.
With that in mind, we wish we could just quickly email you an electronic copy of your MAST Permit. But for the time being, the WSLCB still requires us to mail out paper permits. To start your request for a replacement, just follow the easy, four-step process listed above!
2. You need to change your name or gender on your MAST Permit.
If you have a name or gender change, you must update your MAST Permit to ensure that the information listed on it matches the information listed on your official identification.
You do not need to provide a copy of a marriage certificate or other paperwork verifying the name or gender change, just follow the easy, four-step process listed above!

If your existing MAST Permit is nearing expiration, we recommend you renew it rather than replace it.
